CLAIMS FORM SUBMISSION GUIDELINES
Benefits are valid for a 24 month period from purchase of the vehicle. Customer must be registered prior to the submission of any claims. If you have already registered then simply login and proceed to the claims area to file. If you have not registered yet you may register here All reimbursements are restricted to the registered owner of the registered vehicle for which the Platinum Benefits Portfolio is issued. Original paid receipts, issued in the name of the person which the vehicle is registered to must accompany all claims. All claims must be submitted to Fresh Beginnings, Inc.© and/or any of our affiliates are independent contractors and do not provide any Emergency Road Side Services or Parts.

Take advantage of the money-saving vouchers in your Customer Benefits and Scheduled Maintenance Checkbook. A handy Maintenance Record Schedule will help ensure that your vehicle is always ready for the road. (maximum of four tires during your two-year membership)

Inside your Platinum Customer Benefits Portfolio you will find great saings on vacation getaways, airfare, cruises and car rentals. If it's a road trip you are planning check out the goodies section for travel tips and tools to ensure a safe and enjoyable trip.

Damaged Tire Reimbursement
It never fails-a road hazard destroys your tire at the worst time and place. The burden is less when you know that we will gladly process your tire replacement claim for up to $50 per tire (maximum of four tires during your two-year membership)

Service Call Reimbursement
Wake up to a flat tire? Leave your lights on and killed the battery? Run out of gas? We can't be there to fix the problems, but we will be happy to reimburse you up to $25 of the cost of the service call. (maximum of $200 during your two-year membership)

>Vehicle Towing Reimbursement
If your vehicle becomes disabled and must be towed, we will reimburse you up to $50 per incident (maximum of $200 during your two-year membership). You pay the provider for the tow, send us your completed with the receipts attached or file online and mail in the receipts and we'll send you the check.

Trip Interruption Reimbursement
If you are more than 125 miles from home and your registered vehicle becomes disabled and if you are required to lodge overnight while your vehicle is repaired we will reimburse you up to $50 per day (6 days maximum) for unexpected lodging and meal expenses.

Emergency Locksmith Reimbursement
We will process two emergency locksmith reimbursement claims (up to $25 each) during you two year membership. Enter your vehicle key number on your registration form and in the event you lose your key, we will provide you with the number so you can quickly get another set made.
